Yes. Our service agreement is the contract between our clinic and you. It stipulates the service we agree to provide you with along with rules or guidelines that must be followed within the context of our agreement. This includes things like: Clear communication, honesty, mutual respect, etc. This document also contains your basic personal details (such as name and DOB) along with NDIS information (like your NDIS number and NDIS plan dates).

Currently, Swallow Ability has capacity to complete Swallow Assessments within 2-3 weeks of receiving a completed service agreement.
